Thanks a lot. What you wrote, makes sense.

Have you done this with IRA held securities before? Did it work as you described? For me it's the first time when the securities are in IRA.

Yeah, quite a bit of work for probably small check at the end even if everything is filled correctly.

BTW, I talked to someone from the ACAS litigation office (might have been Citibank person on behalf of them -- I did not catch) and they suggested marking "IRA..." box and just putting myself and my address as claimant. She said that they will know who the custodian is from attached documentation. I still wrote the "FBO Jurgis ... address, custodian" by the IRA box just in case. :)
