Fast forward five or ten years. Without the Americans footing the bill for medical advances things won't get better. Certainly as the Obamacare death panel ratchets down payments pharmaceutical companies and medical device makers will be forced to extract a fair share of the R & D from the rest of the world. Costs will rise in places like Britain. Since there will be no will to weather dramatic cost increases that will necessitate worsening care.
