I extrapolated from his discussion of cost sharing:

"...would be required to provide Medicare beneficiaries an unrestricted cash rebate reflecting most of the difference."

If you choose a provider that charges more than the established rate, you would end up paying more because you would get less back. That pressures price-conscious patients to more "efficient" providers while allows others to choose less "efficient" ones.
