I would suspect that you need:

- name of beneficial owner
- name of person acting as trusee if applicable
- address
- number of ATPG shares currently held
- number of ATPGP shares currently held
- Phone number
- Email Address

Sheets with this info for every holder would probably be attached to the letter.

In my case for example, I have ATPGP in 3 accounts. 2 of them are accounts for family members that I'm the trustee for. Techniocally I count therefore as 3 holders. This is actually good, because in a bankruptcy I believe they look at the number of holders as well as the amount of shares.
