The problem is that there seems to be no way to apply them without human bias playing a big part.

Without a rational underlying standard the bias would be greater. It's easy to be bothered by the negatives but at least the bias has an anchor of sorts. Without it, allowances would be entirely up for grabs.

not for the entire healthcare, but for the safety net.

I don't think that so conspicuous a two-tiered system would be politically feasible in our society, however sensible from a cost perspective.
