Years ago, I read Stephen Ambrose's book on Lewis and Clark, Undaunted Courage. One thing that stuck in my mind from that book is that by the time Lewis and Clark got there, the plains tribes had already been decimated by disease, presumably from incidental and indirect contact with trappers and such. They went through a lot of big, deserted settlements, as I recall, and other communities that were a tenth of what they used to be.

It's just the nature of disease. It's certainly bad to spread it intentionally, but in the long run, the limited intentional incidents were probably inconsequential compared to the natural spread.
