Yes, they are, and the Doctors are bringing it on us with their trade union restrictions. NP,s can be an excellent triage Doc in an HMO environment, but you can get a bad one. I let a NP working at Kaiser as a Dermatologist be my specialist for two years. A mistake. I would not use one again working as a specialist. Well, that's probably going to be the future -- but you may not have a choice when it comes to specialists. I envision a model in which there's one doc, likely hospital system employed, with 3-5 midlevels under him. The difficult or perplexing cases are turfed to the doc technically "supervising" (but rarely directing) the midlevels. We saw it first in anesthesiology and now it's everywhere. This model works most of the time; the American people will have to tolerate that. Those doctors that either don't want to supervise or want to see the full gamet of patients will need to accept a large pay cut in order to maintain that sort of practice.
