I have no direct knowledge of Lancet, but as to similar articles and from what I do know of Lancet's peer review publication process, my answer would be -- Yes, either or both. I have observed Lancet takes care with letters, though not as much as with articles, often requiring they be accompanied by footnotes, professional references, etc. Flattered you would ask, and disturbed that you may have mistaken me for the other Opal who co-authors some XOMA papers. She is not me.
