And I have gotten straight faced arguments from folks claiming that because of "competition" the costs are held in check

Good old "competition" the god machine of capitalism

These folks don't have enough sense to see that competition among entities who have no reason for existing is meaningless

For profit health insurance is an example of an entity that adds nothing to the well being of the country and seeks to extract a profit thru manipulating the social or political environment in which economic activities occur, rather than by creating new wealth.

It is a classic example of Rent Seeking:

...and we all know people who will go to the mat, fighting for their right to exist, and in so doing, prevent folks from getting the care they need in order to enhance their bottom lines

That is a real pre-existing condition
