I can't imagine what that would look like and what the liability profile would look like if done by a private practice physician. If it became bureaucratic the default would be "No!" Maybe a Clockwork Orange-like brain scan? But any government that had that power simply wouldn't issue a gun permit to anyone who wasn't connected politically or paid a bribe.
