Employee Paid 2012 544.80

Employee Paid 2013 same but higher co-pays

Dental (delta dental)

Employer Paid 2012: 1,092
Employer Paid 2013: 1,530

Employee Paid 2012 179.28
Employee Paid 2013 same

What I used in 2012
Medical - 1 visit, 30 minutes. Various prescriptions
Dental - 2 visits to the dentist for 20 minute cleaning and 1 set of xrays.

I would prefer the cash please.
