I heard when she have babysitters she told them 'don't turn your back on him' and you don't think he was mentally impaired ?


Why did she say that? Assuming she did, of course. There are plenty of reasons like he tends to wander outside and get into things he shouldn't. Or maybe injuring himself because he doesn't feel pain. There is no sign he ever had problems with violence.
