One of the basic reasons for higher costs is the higher price paid for services: pbs.org

To really get to the bottom of this you would need to examine the true cost structure of the system. A universal system enjoys many benefits in terms of both costs and outcomes -- it is light years ahead in terms of inclusive access, although some would argue that if you are lucky enough to be a the top of the foodchain, the US system has better access and outcomes at least for those who are better off in the society. Even for those who are better off in the current system, things are only fine so long as you don't mind grossly overpaying and if you believe that it is reasonable for people to lose their health, lose their jobs, lose their health care, go bankrupt and then suffer and die as a normal and somewhat random outcome -- like a form of Russian roulette where you have no idea when something will come along and blow your life away.
