Older geometry processess had breakdowns in the 15-20V range so actually original MOS devices operated at up to 15V such as RCA 4000 series logic.

As geometry has scaled the breakdown voltages has lowered.

Leading edge 0.25/0.35 Micron devices typically operate at much lower voltages and the I/O is specially designed to handle higher voltages as the real world interfaces are at 5V and more recently 3.3V.

Having said all that I agree the 3.2V to 3.3V issue is a non issue from reliability standpoint.
