A lot of people think that if they don't shell out directly, its free. And if you don't pay net taxes, its "free" anyway. I think that indirect payment systems are simply asking for trouble, so I dislike insurance and government single payer. Both drive a cost spiral, because they decouple consumption decisions from the payment process. If instead, you pay significantly out of your pocket each time you consume, you tend to keep your brain engaged as to whether its beneficial or not.
