We need some lawyers on this.

1. Ask the receptionist what will happen if you refuse to sign.
2. She will tell you they won't be able to provide services.
3. Write her name and the date in the margin next to your signature, with a note that you are being forced to sign under duress, and that you signed only because it would put your health at risk not to.
4. Then, Law suit.
