You have to enter stuff like name, address, birth date...

A lot of people feel that that's private information and being told that they shouldn't expect privacy when entering might not go down too well.

Other than that under federal HIPAA legislation, demographics such as Name, Address, SSN, DOB, Gender, etc., are PHI as in "Protected Health Information," I suppose you have a point ...
