At least half are fraudulent, IMO. Best way to handle is to "bulk grant" the states and let them figure out the best way to do them. The key to all welfare programs is, "the closer to the recipient, the better it is handled." I suspect a catastrophic Ins setup with a medical credit card would be the best way.
