From my insurance company letter cancelling my plan and offering new packages:

"As of 1/1/2014 the Affordable Care Act (ACA), also known as Obamacare, requires carriers that market to small businesses and individuals must include ten essential health benefits (EHB). One of the ten EHB is pediatric dental benefits.

As a result, you must have EHB Pediatric Dental Coverage to renew your health insurance.

Please review the new packages and options. The plans labeled "w/PD" include coverage for EHB pediatric dental services as required under the Affordable Care Act. The plans labeled "no/PD" DO NOT include coverage for EHB pediatric dental services as required under the Affordable Care Act. Therefore a separate EHB pediatric Dental Plan MUST be obtained in order to subscribe. Please see the EHB Pediatric Dental Instruction document within for more details."

So it appears that my plan was canceled because I was not covered for pediatric dental. Of course, I have no children of pediatric age, but that doesn't matter to the statists who came up with this scheme to kill the US healthcare system.

You refer to "competition from the groups", but can you explain exactly what these competitive factors are? Are people actually checking prices of services? I think most are not. While many doctors appreciate the opportunity to get away from the administrative burden, I've worked with a significant number who have tried it and did not like it.

And I've worked with a good number who felt pressure to move into a larger group organization and just chose to retire instead.

Anyone who argues that the health care environment with respect to physician providers is getting better simply doesn't understand what is going on with them. I've not met one, NOT ONE, who supports Obamacare. I know some do, but I know a lot of doctors and I've not heard one yet speak in a complimentary way about it. Many are scared shitless because unlike most Americans, they do realize we haven't seen the bulk of the changes yet.
