You know you picked a very poor example to show that selling excess capacity of something may not be good business if you don't pay attention to "marginal cost". It's easy to see that marginal cost of something like a hotel room may be a significant portion of the rent you receive. But an airliner with empty seats unsold seats has little extra in marginal cost save maybe extra baggage handling and a few more bags of peanuts.... They don't just clean the seats that were occupied, they clean the whole plane. A sporting event or show eats those empty seats... On Broadway there is the TKTS booth that sells left over seats right up to 30 min. before showtime.... If a doctor can squeeze more customers into his working day, it's essentially all profit except that the payment he receives per patient may be less than he would like to get....he isn't working any harder if he's putting in the same 8 hour day....nor is his staff.... I just read a survey that showed that 55% of docs work 30-40 hours per week...ie they are not overworked.
