I find that extremely difficult to believe.

"Any" is just too broad a claim... (especially knowing that Medicaid is a joint federal/state program, and several of the states run things quite differently, including introducing elements of HMO-like risk management).

Possibly you have bought into a categorical position that will not stand in the light of day.

Sure, 'pay for services' is still a common Medicaid SOP, but it is definitely not the *only* one these days.
