The term spread more widely in popular English as a markedly derogatory term for a person who behaves stupidly. Because of its pejorative connotations in popular speech, health-care workers have mostly abandoned cretin.

Some few people like to use the word "cretin" in a pejorative way to insult and degrade others, most often toward those who disagree with their own points of view. It's very unlikely that the term was used to suggest a thyroid hormone deficiency.
