There should be systems in place to help police officers avoid these situations, and definitely systems of accountability. But, some things in the communities some of these officers police, are problematic- and beyond their control. How do they tell, for sure (when their lives depend on this) who is dangerous and who is not. That might not apply in the Brown case- I don't know, but the whole "unarmed" thing is crap- because you don't know that up front. Much of what police have to go on is demeanor.
