It is sort of obviously not a good metric.

Aaron Caroll tried really hard to make the case the adjustments don't matter. But you can't get past some of the others, like infant mortality, where we save more premature babies than any country in the world yet have a high infant mortality rate because we count extreme premature births as "live" births. So, yeah, we lose a lot but we save a lot.

Lies, damned lies and statistics.
