Offer more to the consumer and at rates they can choose. The many mandatory inclusions would now become elective for a price. Some wastes would not be covered at all. People can have all they want for the varied premiums. Pushaways can be substituted for many ailments. Type 2 diabetes can be controlled by exercise and diet. If you are too fat, you eat too much. If a doctor amputates the wrong leg, there can be a modest settlement , but the doctor will go to prison, not simply have an award paid by an insurance company. People with dangerous vices would have to pay greater premiums, that goes for overeaters, lazy people and drug dependent people. So get that gym pass or get busy.
