It really doesn't work like that. You can cut the quality of the service only so much and still comply with professional standards and pay the malpractice costs. What I see happening now with our customers is that providers are handing these patients off to nurses. Which is probably a reasonable way to proceed under the circumstances, but no one should mistake it as anything other than a reduction in the quality of care for Medicaid (and Medicare) patients in exchange for providing free care for others.
Nurses cannot legally practice medicine, that would lead to malpractice suits....your state has a high percentage of Physician Assistants who can, to some degree, practice medicine...... I think what is typically done to boost revenue is to increase the number of office visits by reducing the time spent or increasing the number of office hours.... An office visit of 5 minutes allows for a lot of customers....... I don't hear of it being an issue around here.
