Under our Constitution, revenue bills must originate in the House.

The S. court has ruled that the gut and amend procedure can be used where the revenue raising provisions are incidental amounts or are raised for the purpose of raising revenue for the legislation that is the subject of the bill.

The Court has not considered whether the procedure can be used where vast sums of money are raised which are not specifically dedicated for the purpose of the legislation, in this case, healthcare. In the ACA, funds raised by the law are dumped into the general fund.

Really, if the clause doesn't apply in this instance the language is totally superfluous, and the Constitution isn't known for superfluous language.
