The engineer first said he was trying to slow the train. Then he got a lawyer and went blank. Looks to me like he had mental issues and was trying to take people with him to the grave.

An attack is possible, and certainly all possibilities must be explored. Right now I am going with the Occam's Razor explanation - human error. The engineer f*cked up. Happens to people with a lot more experience than he had.
