Free for Medicaid beneficiaries, which are the substantial majority of those who have received coverage under Obamacare. The number who have received actual insurance, which they pay for some portion of, is relatively small.

Medicaid beneficiaries pay zero, or in a few cases, a small (e.g., $10, copay on office visits and have no deductible at all as 100% of costs are paid by taxpayers).
