No one's health should be subject to 'market forces'. Selling life saving drugs for 'what the market will bear' is simply evil, since a person will spend all they have to stay alive and healthy.

I realize pharmaceutical profits enable these drugs, but the system we have is morally wrong. I don't know what the answer is, but our current system is not it. I'm sure as we progress, people will view our current system as crazy.

Giving out free samples is considered marketing.

Also its not like their is a fixed budget and marketing takes away from R&D. The idea of marketing is to raise overall demand and increase revenue. To the extent that happens the more that gets spent on marketing the more money is available for R&D. To the extent it doesn't happen its a problem of poor marketing.
