If it is an actual war crime (which I consider unlikely but it isn't impossible) then someone is responsible for that. If its a screwup then someone is responsible for the screwup. Depending on the nature of the screwup the nature of the screwup, and whether any policies were violated in committing the screw up, the proper response could range from just criticism and analysis of the mistake, to relieving them of command, to prosecution (for violating orders and policy if it was done, not just for making a mistake).
