BioM, if the doctors use it in first line setting and the company agrees with the pricing, doesn't the insurance have the last say since insurance company always wants to find a way to avoid payment? I know it can be used for off label treatment protocol but I just don't know how widespread the practice is. What is the design difference between first line and second line clinical trials? Is it just the number of patients and the length of time? TIA.
