Here's my take, as they say in the current vernacular:

"an" before non-aspirate "h". "an honor"
"a" berore aspirate "h". "a hotel"
Exception: words pronounced on other than the first syllable. "An historic..."
BUT, I never heard of anyone who had "an hysterectomy" so the rule must be variable, or maybe I have the rule wrong. Maybe it's just "an" with words accented on the second syllable only.
