Samsung had a design problem in the sense that W. Edwards Deming would use. Which means a properly designed product cannot physically be assembled improperly.

One of Samsung's many Chinese suppliers of battery cases built cases where the tolerance was too large by one millimeter. Unfortunately the battery could still be assembled with this defective part.

The resulting short would be created by random as a battery was charged and a finger of lithium would form around the separator which was the correct size in a case which was 1 mm too large.

Making integrated circuits or LEDs the tolerance levels are measured in nanometers, which it why they're made in so few places. Places and people who have long experience in meeting that level of accuracy. It's not an easy thing to recreate, even if you buy the same production equipment from Applied Materials that everyone else buys.
