It's not perfect, but it's probably reasonably cheap and clear and easy.
You described what most Americans had 50 years ago. Major medical covered fairly well, zero coverage for minor, routine and preventative treatments. HMOs started around 1970 with the idea that by providing free preventative care, they could avoid the big expensive problems. It hasn't worked out that way, let's try something different.
