Trump's hand is clearly mimicking Serge Kovaleski's withered hand. The statement "You've got to see this guy" was a clear reference to his appearance. Typical shallow Trump personal attack. Furthermore, after he was criticized for mocking the reporter, Trump lied and said he didn't know the guy and had no idea what he looked like. In fact, Kovaleski has been a reporter for decades in NYC, has interviewed Trump in his office and met him at press conferences. They were on a first name basis for a long time. And the only reason for this is that the guy had written a story 14 years before which doesn't support Trump's claim.
