Rack rates are actually paid by rich people without insurance and some foreigners visiting the U.S., a small minority of patients. But treating a patient who has insurance that the doctor "takes", restricts the doctor to the reimbursement rate contracted with the insurer.

There is no writeoff of the excess, and your saying so shows how ignorant you are.
