It is single in the sense that the rates are set by the government and provides subsidies for people of low incomes. The rest is an administrative detail.

The doctor who provides the health care should make a profit. He does the work. The insurance company is acting as the middle-man and should not be trying to cut itself a profit from the transaction. Is that simple enough for your capitalist mindscape?
