All forms of insurance, at least all forms that cover large numbers of people, government or not, deny claims and treatments. Resources are not infinite, and some claims are fraudulent.

Non-profits still have some incentive to control costs. Let cost inflate too much and they fail, or at least take a hit until they can raise rates. To the extent they have less incentive to control costs that just makes the overall costs of medical care higher.

You and Tim might be talking past each other. I almost never agree with Tim, but there has to be a means of denying treatment - and claims. Fraud is an obvious justifiable reason for denying claims. But we better also get use to denying treatment or the health care system as we know it will implode on itself. Does that 95 year old really need a new knee? If the drug we are taking cost $250,000 a year and has barely perceptible advantages - do we always have to pay? Other countries have means of limiting care - they have to. Only in America where insurance companies are reimbursed no matter what the cost do we almost never refuse service. We need an honest discussion on this - not one pounding our ideologies.
