Sure wharf; but the dr is often looking at the issue without the restriction of what it cost. More than once I have had my dr inquisitively ask or ponder if my insurance paid for a drug or procedure. It isn't nearly as clean as you seem to suggest it is. Take a boob job dr; do they ever say you don't need new boobs? They bring out pictures and ask what size and shape. .......Other drs have gotten in trouble because of sweetheart deals with drug dealers pushing their products.
