maybe someone suicidal out of the desert

The most likely current scenario for me would be a suitcase nuke partly sourced from places like N.K. or Pakistan, on a multi-day timer, and placed & shielded so as not to be easily detected by "normal" radiation scans. Perhaps it'd even be assembled on site, and would likely not have a yield in the mega-tons.
