There's no reason why a simply law can't be passed that requires every insurance company to offer plans in every state. It would take five minutes to craft such a plan. Competition lowers prices.

Why didn't the dems do that when they controlled congress and passed the ACA?

All this was well known.

healthcare.gov explicitly stated there was NO CAP on rate increases.

Nearly every Dem on SI pretty much was in denial of the reality that the ACA was crafted by the for profit health care industry and looted $700B from medicare.

The Dems had power for only four years, two as the Great Recession began and two to fix the damage caused by the Great Recession.
Are didn't you answer the question? Knowing how screwed up the ACA was, why didn't the Dems do a 5 minute fix?
