The problem/solution vs predicament/response thing is similar to my longstanding hobby horse about requirements and design where we address design flaws rather than taking a deep dive and satisfying the underlying requirements. But we have no mechanism for addressing the underlying problem. I've seen it done on a micro level. I was trained in facilitation for consensus by the folks who were with Carter at Camp David. I have personally been party to minor miracles in getting consensus from wildly disparate and strong perspectives. It's a beautiful thing when it happens. But the technique doesn't scale up.
