I appreciate your incites.

I agree that culture is a big part of it. In the end, nobody is forcing relatives to agree to prolonging the suffering of a dying individual by subjecting them to last hour medical procedures. It is just that most seem to spend the majority of their lives in a perpetual state of denial when it comes to death. And when the time comes to make the tough decisions, people are not prepared so they let the medical system decide for them.

And then the medical system makes the decision based on $$'s. Sure most Doctor's have good intentions, but most are also not trained to manage death. So instead they recommend and do what were trained to do which is to administer medical treatments.

Personally, I think the medical system needs to take the lead here on helping to change the culture (against their economic best interests) and training doctors and other staff to manage death and help coach people and their relatives through death.
