But these folk need to be physically strong. A bad heart valve or a severe allergy would likely kill a rural baby, whereas a middle-class suburban baby would get treatment.
When you visit relatively poor agrarian regions without a Kaiser around the corner, the selection has already taken place early.

Farmers and steelworkers can't telecommute. Maybe this is very curmudgeonly of me, but I see any job that can be done over a modem as not necessary for survival. Such jobs, while they undeniably create all sortsa wealth, are the stuff which builds culture on top of subsistence.
Of course, in a hundred years, all ag and manufacturing might be done by smart machines and hobbyists. So the whole premise is all fluxed up. :-)
