i have a very basic question for anyone that would like to field it.

i have been watching the tick closely on a few of my favorite stocks and i have been disturbed by some trades(usually a bigger block) trading outside of the bid/ask.
for example: bid 40 ask 40 1/2 10000 shares trade at 39 1/2 or 40 7/8

why? how can you tell if its a buy or sell if its outside of the bid and ask?
