Scott, thanks, this is much easier than Eddy's 10-point program.

I think to a great extent our "argument" is a half-empty/half-full type of thing, and a lot depends on the time frame, say, to get those peer-reviewed papers published to establish that "majority" bit. What is encouraging is their (Blue Shield's) willingness to follow the opinion of respected independent entities as listed in the "Standard Reference Compendia."

The information posted over the last few days may enable one to formulate more educated guesses about the likelihood of reimbursement for specific cases.

Harold, the "recap" site is a great resource if not always up to date; I do not know of a better one where you can get so much in one place.

The exact same language regarding off-label use appears in Group Health Cooperative's benefit pamphlet. (GHC is a large HMO in the Pacific Northwest.) So, as you say, this language is probably typical.
