Two types of Instinet: broker and institutional
Broker allows brokers to cross electronically. Brokers who don't make a market can use it to place a bid for stock, instead of taking the offer when they want to buy or vice versa if they want to sell.

Institutional is very important. It allows institutions to trade with each other without going through a regular broker. They use this sometimes for after market trades, but usually to get in the spread. Ex. if ABC stock is trading at 8 1/2 by 9, a mutual fund could bid 8 3/4. Another institution who wants to sell may have an offer of 8 3/4. Instinet crosses the two. In this case both get their traded done with a savings of 1/4 of a point versus going through a broker.

Instinet fees can be as low as 2 cents per share, but you have to do a lot of shares to get that price.
