I guess something massive would get them in trouble. Many years ago, I was a member of a large medical group of maybe a couple of hundred docs - and we had a “total risk” deal with Medicare. We paid hospitals, doctors, everybody. I served (regrettably, unpaid) on their board. We used to purchase re-insurance - for extremely costly cases. I’d expect that health insurance companies have similar arrangements, for situations when expenses get out of hand.
