Some people will die in the first week, most (as Wuhan doctors find) in the 2nd, some later. So, you still use approximations. Dividing up to date deaths by the number of up to date confirmed cases, over time you get closer to the real case fatality numbers. The big unknown, IMO, is the prevalence of very mild cases, who may never get diagnosed. They’ll be the big spreaders.
